脂肪がお腹 (なか) の周りにたまると「apple shape (アップル型)」、腰の周りに付いてしまうと「pear shape (ペア型)」の肥満体型になる。どちらの体型かの判断は「waist-to-hip ratio (ウエスト・ヒップ比 WHR)」が決め手になる ( WHR≧0.82; apple shape, WHR＜0.72; pear shape)。
ところが、Oxford大学の Dr Sanne Petersらの研究グループは、研究プロジェクト「UK Biobank」に登録されたイギリス人 50万人 (40-65歳) の医療データを解析し、「アップル型」の女性は心臓発作 (heart attack) の発症リスクが高いことを明らかにした。
"The report said that body compositon and fat distribution differed markedly between the sexes, with women having a predominance of fat mass and subcutaneous fat and men having lean mass and visceral fat."
[ 研究によると、男性と女性では身体組成も脂肪分布も全く違っていて、女性は体脂肪、皮下脂肪が断然多く、男性は筋肉質で内臓脂肪が多い。]
このため、単に肥満度を表わすBMI値 (体格指数) だけで、心臓病の発症リスクを判定できない。女性は BMI値が高く、WHR値も高いと、男性に比べて圧倒的に心臓発作のリスクも上がることが分かったという。
なお、NHS Choicesによると、「Heart Attack syndromes (心臓発作の症状)」は次のとおり。
・chest pain - a sensation of pressure, tightness or squeezing in the centre of your chest.
・pain in other part of the body - it can feel as if the pain is travelling from your chest to your arms (usually the left arm is affected, but it can affect both arms), jaw, neck, back and abdomen.
・feeling lighheaded or dizzy.
・sweating.
・shortness of breath.
・feeling or being sick.
・overwhelming sense of anxiety (similar to having a panic attack).
・coughing or wheezing.
・胸痛：胸の真ん中辺にに圧迫感、こわばり、締め付け感を覚える。
・胸部以外にも痛みがあり、痛みは胸から腕 (ふつうは左の腕だが、両腕が痛くなることも)、あご、首、背中、お腹へと移って行くように感じる。
・頭がふらつく、あるいは目まいがする。
・発汗
・息切れ
・体調不良
・どうしようもない不安感 (パニック発作によく似た症状)
・咳、喘鳴 (ぜんめい)

"Although the chest pain is often severe, some people may feel only minor pain, similar to indigestion, in some cases, there may not be any chest pain at all, especially women, the elderly and people with diabetes."
[ 激しい胸の痛みを覚えることが多いが、人によっては消化不良程度に治まり、大した痛みを感じないこともある。とくに、女性や高齢者、糖尿病罹患者の場合には、まったく胸の痛みを感じないケースもある。]
つまり、「アップル型」の女性は、自分で気づかないうちに心臓が弱まっているのだ。病院の診察でも、女性については心臓発作の症状・リスクを見逃すことが多いという。
なお、Dr Petersらの研究結果の詳細は、医学雑誌「The Journal of the American Heart Association」に発表された。
